You are responsible for any mobile data fees and other third-party charges you may incur in connection with your access, download and use of the Applications.
myBlue Mobile Application - The myBlue Mobile Application includes features to support Member access to various components of your Health and Wellness Benefit Plan from Blue Cross & Blue Shield of Mississippi. This includes Benefits & Accumulators, Medical and Prescription claims processing, Formulary drugs search, Providers Search, Virtual ID card, and other information related to Wellness, along with various features for account maintenance including registration and password updates.
Connect Blue Mobile Application - In connection with Color Me Healthy! and Blue Primary Care Home, the Connect Blue Mobile Application includes features which give you access via video chat to a Wellness Coach who will provide wellness coaching, as well as follow-up wellness services.
User Provided Information - User registration is mandatory in order to use the basic features of the myBlue Mobile Application. The myBlue Mobile Application obtains the information you provide when you download and register the Application. When you register with Us and use the myBlue Mobile Application, you generally provide: (a) your Subscriber ID, name, SSN, date of birth, user name, password and other registration information; (b) information you provide Us when you contact Us for help; (c) information you enter into Our system when using the myBlue Mobile Application, such as contact information. We may also use the information you provide Us to contact you from time to time to provide you with important information regarding your Health and Wellness Benefit Plan and required notices. Connect Blue registration is not required; however, you must register to use myBlue through www.bcbsms.com or the myBlue Application before you can use Connect Blue. If you do not register for Connect Blue, none of the information listed above is obtained, used or retained by us when the Connect Blue Application is used.
Automatically Collected Information - In addition, the myBlue and Connect Blue Mobile Applications may collect certain information automatically, such as the type of mobile device you use, your mobile device's unique device ID, the IP address of your mobile device, your mobile operating system, and information about the way you use the Applications.
The myBlue Mobile Application does collect precise information about the location of your device. If you choose the "Use Current Location" option on the Providers search feature in the myBlue Mobile Application, the myBlue Mobile Application will utilize Location Services to identify the closest Network Providers. Otherwise the default option is to lookup Netwok Providers by inputting ZIP code. This information is not shared with third parties. This information is only accessed to provide the services within the myBlue Mobile Application that you choose to use.
To use the Connect Blue Mobile Application, you must authorize access to your mobile phone's camera and microphone. Your camera and microphone will only be used for wellness coaching and access must be authorized each time you use the Application. In addition, your camera and microphone will only be accessed during the duration of the wellness coaching session and will be automatically disconnected at the end of the call. Access will not be used for other monitoring purposes.
The application does not support advertising and data is not obtained by an advertising network or other third party for the purpose of advertising targeting.
You may at any time opt-out from further allowing Us to have access to your location data by disabling Location Services for the Applications in your mobile device settings.
Opt-out of all information collection by uninstalling the Applications - You can stop all collection of information by the Applications easily by uninstalling the Applications. You may use the standard uninstall processes as may be available as part of your mobile device or via the mobile application marketplace or network.
Data Retention Policy, Managing Your Information
We will retain user provided data for as long as you use the myBlue Mobile Application and for a reasonable time thereafter. Please note that some or all of the user provided data and tracking data may be required in order for the myBlue Mobile Application to function properly, and We may be required to retain certain information by law. We do not retain any information from your use of the Connect Blue Mobile Application.
We do not use the Application to knowingly collect data from or market to children under the age of 13. If a parent or guardian becomes aware that his or her child has provided Us with information without their consent, he or she should contact Customer Service at 1-800-942-0278.
Our privacy practices are intended to comply with the Health Insurance Portability and Accountability Act (“HIPAA”). We will maintain the privacy of your personally identifiable health or medical information (“Health Information”) as required by HIPAA and the regulations promulgated under that Act. We encourage you to review our Notice of Privacy Practices, which describes how we may use and disclose your Health Information. Your Notice of Privacy Practices may be found within your Health & Wellness Benefit Plan or by visiting www.bcbsms.com.
Disclosures in Accordance with Law
We may disclose user provided and automatically collected information: (a) as required or permitted by law, including complying with legal process (for example, we may disclose your information as necessary to comply with an authorized civil, criminal, or regulatory investigation); and (b) when we believe in good faith that disclosure is necessary to protect Our rights, protect your safety or the safety of others, investigate fraud, or respond to a government request.
We are concerned about safeguarding the confidentiality of your information. We provide physical, electronic, and procedural safeguards to protect information we process and maintain. For example, we limit access to this information to authorized employees and contractors who need to know that information in order to operate, develop, or improve our Applications. In addition, your camera and microphone will only be accessed during the duration of the wellness coaching session and will be automatically disconnected at the end of the call. Please be aware that, although we endeavor to provide reasonable security for information we process and maintain, no security system can prevent all potential security breaches.
The safety and security of your information also depends on you. We urge you to take precautionary measures in maintaining the integrity of your personal information and to activate the security settings on your phone. Please be responsible in making sure no one can see or has access to your personal account and log-in/password information. Please exercise caution and use sound judgment when using the Applications. If you use the Connect Blue Application in a public environment or where others may hear or see your wellness coaching session, such use is at your own risk.
By using the Connect Blue Application, you are consenting to the use of your mobile phone's camera and microphone during the duration of your wellness coaching session. Each time you log into and use the Connect Blue Application, you will be asked to provide your consent. If you choose not to provide consent, access to the Connect Blue Application will not be granted and your wellness coaching session will not commence.
